Dr Daniel Yap is a partner of LifePlus Medical Group.

Dr Yap graduated from Yong Loo Lin School of Medicine, National University of Singapore in 2010. He also holds a Graduate Diploma in Family Medicine (GDFM) and is a Member of the College of Family Physicians Singapore.

Upon completion of his undergraduate studies, Dr Yap trained under the internal medicine residency programme in Tan Tock Seng Hospital, where he underwent rigorous rotations in core disciplines such as General Medicine, Geriatric Medicine, Endocrinology, Respiratory Medicine, Intensive Care Medicine, Cardiology, Infectious Diseases and Dermatology.

This allowed him to gain a breadth of clinical skills and knowledge across a variety of disciplines. He also underwent a posting in Emergency Medicine, where he acquired skills and knowledge in acute care.

Following his stint in the public sector, Dr Yap went on to serve the community of Punggol Central as a Family Physician with a reputable healthcare chain. During his 4 years in the community, Dr Yap was able to practise the skills he had acquired during the myriad of clinical training. Notably, he also gained confidence in serving patients of various age groups, especially the paediatric population which had formed a substantial part of his patient base.

Dr Yap practises a holistic approach to healthcare and believes that Family Medicine and Primary Care are crucial aspects of our national healthcare system. The doctor-patient relationship built on mutual trust and respect is what keeps him going day to day.

Dr Tan Weiming

Dr Tan Wei Ming is a partner of LifePlus Medical Group.

Dr Tan graduated from Yong Loo Lin School of Medicine, National University of Singapore in 2010. After completing his undergraduate studies, Dr Tan had the opportunity to gain a valuable breadth of clinical skills and insights from working in various medical and surgical specialties across reputable healthcare institutions in Singapore. As a junior doctor, he rotated through postings such as Obstetrics & Gynaecology at KK Women’s and Children’s Hospital, Orthopaedics at Singapore General Hospital and Internal Medicine at Changi General Hospital.

Dr Tan subsequently worked in the primary care sector in Singhealth Polyclinics and National Healthcare Group Polyclinics, before returning for a short stint in the Infectious Diseases department at the Communicable Diseases Centre and the Clinical Standards and Improvement department, at Tan Tock Seng Hospital.

Dr Tan went on to obtain a Graduate Diploma in Family Medicine (GDFM) and worked for a few years as a Resident Physician in Singhealth Polyclinics, providing quality, cost-effective, and efficient primary healthcare services to the community. Despite having left his position at Singhealth Polyclinics to be a partner at our group, Dr Tan still returns to polyclinics from time to time to serve the community and keep abreast of the latest progresses in the public sector healthcare. This enables him to serve patients to the best of his abilities on both sides.

Dr Tan Jinxuan

Family Physician

Dr Tan Jinxuan is a family physician at LifePlus Medical Group (Sin Ming) and LifePlus Medical Group (Bedok). 

After graduating from Yong Loo Lin School of Medicine, National University of Singapore in 2013, he worked in the departments of internal medicine, general surgery, and emergency medicine in Tan Tock Seng Hospital where he had the opportunity to manage a broad range of acute conditions under the tutelage of respected senior physicians.

Dr Tan then spent several years practising family medicine in National Healthcare Group Polyclinics where he provided holistic care to patients of all ages. He is passionate about providing quality primary healthcare services, attaining a Graduate Diploma in Family Medicine (GDFM) in 2019. He has been practising as a certified family physician ever since and has a keen interest in the management of chronic medical conditions. He is also an appointed Clinical Teacher with the Lee Kong Chian School of Medicine.

Dr Tan is trained in performing minor surgical procedures such as excision of common skin lumps and skin tags, steroid injections for finger and wrist tenosynovitis, and surgical management of ingrown toenails. He had led the minor surgical procedure service in the polyclinic previously and he is keen to make such procedures more readily accessible to the community he is now serving. 

Outside of work, he enjoys reading, running, and spending time with his wife and two daughters.

Dr Kee Soon Chuan

Dr Kee Soon Chuan is a resident physician at LifePlus Medical Group (Hougang Central).

Dr Kee graduated from Yong Loo Lin School of Medicine, National University of Singapore in 2006. 

Dr Kee had previously trained in various departments in Singapore General Hospital including emergency medicine, orthopaedic surgery, anaesthesia, internal medicine, and cardiology.

Dr Kee was previously practising as a resident physician in the 24-hours clinic of two established private hospital where he provided quality and efficient acute medical care.

Dr Kee hopes to bring his experience to the community level and provide quality care to all his patients.
